Question: I would like to customize the default Patient Information and Medical History form provided by OperaDDS.
Answer: Each offices is provided with a list of default forms. Some can be customized (edited) others cannot be edited.
Patient Information (Non customizable because the PDF that is generated is aggregated for a better office experience.)
Medical History (Non customizable because the PDF that is generated is aggregated for a better office experience.)
Office Policies (Fully Customizable by the office within their forms editor)
Financial Policies (Fully Customizable by the office within their forms editor)
HIPAA (Fully Customizable by the office within their forms editor)
All forms have an edit button which will say if the office can edit the form. If it is available, they can edit the form.
